Report on collision between a road-rail machine and a Robel 25 Track Vehicle close to Øvre Vang level crossing on 9 February 2021

On Tuesday 9 February 2021, a collision took place between a road-rail machine and a stationary Robel 25 Track Vehichle close to Øvre Vang level crossing between Jevnaker and Grindvoll on the Roa-Hønefoss line. The driver of the road-rail machine sustained minor injuries in the accident. There was also material damage to the road-rail machine’s excavator equipment and rail wheel system. The Robel 25 derailed in the collision and the driver compartment sustained damage. The cause of the collision was that the road-rail machine was operated in a way that disabled the breaks. This was due to inadequate identification of competence and training needs on the part of the contractor Spordrift AS and the infrastructure manager Bane NOR SF. Spordrift AS was certified according to NS-EN ISO 9001:2015 Quality management systems. The certification had been issued shortly before the accident by the certification body KIWA AS. The Norwegian Safety Investigation Authority's (NSIA) investigation has shown that despite the certification, the management system was inadequate in key areas. The investigation has also shown a potential gap between the industry’s expectations of such a certification and what the certification bodies provide.
The NSIA issues two safety recommendations on the basis of the accident. Bane NOR SF is requested to follow up whether the suppliers have adequate systems in place to ensure competence and training that supports Bane NOR SF’s safety policy. Furthermore, the NSIA recommends that Norwegian Accreditation evaluate whether its procedures for granting accreditation to certification bodies in the railway industry are sufficient to achieving the purpose of an ISO certification.

The immediate cause of the collision was that the road-rail machine was operated in a way that put its safety systems out of action. Bane NOR SF’s supplier Spordrift AS had not identified the necessary skills required to operate the machine, nor carried out any systematic training in the machine’s functions.
The Norwegian Safety Investigation Authority recommends that the Norwegian Railway Authority request Bane NOR SF to follow up that suppliers have adequate systems in place to identify competence needs and provide appropriate training to ensure compliance with Bane NOR SF’s safety policy.

Shortly before the accident, Spordrift AS had been issued an ISO 9001 certificate as a full service supplier of construction, operation and maintenance services for railway-related infrastructure. However, the management system was not an effective means of identifying competence needs and thereby training needs in the company.
The Norwegian Safety Investigation Authority recommends that Norwegian Accreditation evaluate whether its procedures for granting accreditation to certification bodies in this industry are sufficient to achieving the purpose of an ISO certification.
