Report on derailment on the corner of Parkveien and Welhavens gate in Oslo on 22 January 2021

On Friday 22 January 2021, tram 147, which operated the line 18 service between Rikshospitalet and Grefsen station, derailed when it turned into Welhavens gate from Parkveien. No human injuries occurred, but the track was closed for repair and could only be opened later the same day. The main cause for the derailment was that the track was worn outside its wear limits.
The NSIA investigation has focused on Sporveien Trikken AS' systems for maintenance and control with the track, and how findings and deviations were handled to ensure that the quality of the track was sufficient for safe traffic.
The investigation has shown that conflicting goals, together with demanding priorities, led to inadequate maintenance at these locations.
During the year since the incident, Sporveien Trikken AS has identified a number of follow-up points and strengthened the organisation so that track maintenance is carried out as intended. The NSIA considers that these measures support Sporveien Trikken AS' safety policy and will provide a better basis for priorities when goal conflicts arise.
The Norwegian Safety Investigation Authority does not issue any safety recommendations as part of this investigation.
