Railway report 2015/07

At 03:57 on Monday 3 November 2014 CargoLink AS' southbound freight train 5932 derailed at Points no V007 on track 1 at Eidsvoll station while en route to Alnabru on the Dovrebanen line. On passing the points, the locomotive shook in a way that was not normal, but without derailing. The driver engaged the emergency brake, but the first three rail cars had passed the points before the train stopped. These three rail cars derailed. Track 1 had been reopened for traffic that morning following alterations and extension work in the station area, and train 5932 was the first to use the track after it was reopened.

Safety recommendation

Safety recommendation JB no 2015/07T

On Monday 3 November 2014, freight train 5932 derailed from track 1 at Eidsvoll station because the points were incorrectly set. The track had been reopened that morning after alteration work in the station area. Checklists are prepared showing which components and functions are to be inspected in order to ensure that the system functions correctly. The person responsible for the final inspection at the traffic control centre was in possession of the test protocol and filled in the checklists, while an assistant was out in the field. The assistant did not have a copy of these documents. In this case inspection of the position of the points was overlooked.

The Accident Investigation Board Norway recommends that the Norwegian Railway Authority recommend that the Norwegian National Rail Administration (Jernbaneverket) introduce requirements to provide assurance of redundancy and barriers so that checkpoints are not overlooked in connection with the performance of function tests and final inspections.

Safety recommendation JB no 2015/08T

On Monday 3 November 2014, freight train 5932 derailed from track 1 at Eidsvoll station because the points were incorrectly set. The track had been reopened that morning after alteration work in the station area. The track and interlocking system had been inspected and approved, but, in this case, the physical inspection of the position of the points was left out as a result of a misunderstanding. Jernbaneverket has not established fixed phrases for communication between the person responsible for the final inspection and assisting personnel.

The AIBN recommends that the Norwegian Railway Authority recommend that the Norwegian National Rail Administration (Jernbaneverket) introduce fixed phrases for use in connection with testing and inspection of safety-critical functions related to the interlocking system.
