Report on marine accident - fall over board from the wellboat Øysund 16 November 2017

On the 16. november 2017 it was reported from the Norwegian rescue services that a person fell over board from a fish carrier south of Haugesund, Norway. The investigation has shown that the wellboat company had not taken necessary measures to reduce the risk of traffic on the main deck in rough sea. The AIBN submits a total of two safety recommendations as a result of this investigation.

The accident happened when a person walked on the main deck in rough sea. The person most likely fell overboard as a wave swept over the main deck. The person was found and rescued by the crew in the vessel's MOB boat, and transported to Haukeland Hospital by helicopter. The person was decleared dead the following day.

The storm bridge onboard did not meet the requirements of the Load Line Convention, and the crew did not experience it as safe to use in rough sea conditions. The Norwegian Maritime Directorate has informed that they are preparing a circular on this issue and that they will have more focus on enforcing the relevant
provision.

Safety recommendation MARINE no 2018/04T

The accident with Øysund on 16 November 2017 occurred when a person walked on the main deck in rough sea. The person most likely fell overboard as a wave broke inwardly. The investigation has shown that the wellboat company had not established documented guidelines or routines for the use of means of communication or personal safety equipment when staying or working on deck.

The Accident Investigation Board Norway recommends Sølvtrans to undertake a documented risk assessment of the dangers of traffic on the main deck and implement procedures and guidelines for this on board.

Safety recommendation MARINE no 2018/05T

The investigation of the accident with Øysund on 16 November 2017 has shown that the wellboat company’s MOB procedure was not sufficiently implemented. There were problems with triggering the general alarm and the MOB-button in the electronic chart was not activated. The MOB boat was only manned by one person and there was no radio communication between the MOB-boat and the bridge. The MOB boat had no safety equipment on board that could be of help in getting the person out of the water.

The Accident Investigation Board Norway recommends Sølvtrans to revise the procedures and equipment for MOB-situations and MOB-exercises, and carry out realistic exercises in line with the company's revised plans and current requirements.
