Report on the foundering of fishing vessel Stian-Andreoutside Øksfjord on 1 October 2020

The fishing vessel Stian-Andre was approaching Øksfjord when the skipper fell asleep and collided with a fish farm. The collision resulted in a rift on the starboard side and the vessel took in water. The crew moored the vessel to the quay and left the vessel safely. As the vessel was gradually filled with water, the ropes eventually snapped, and the vessel sank. The skipper navigated with an automatic navigation system that steers the vessel along a predetermined route. The system did not require any actions from the skipper. The NSIA's assessment is that manual navigation when approaching port, would be a barrier to prevent falling asleep. Manual navigation requires more attention compared to automatic navigation.

The NSIA believes that the reason the skipper fell asleep was a combination of three different factors; sleep deprivation, fatigue due to time of day and sleep inertia.

No safety recommendation is issued in connection with this investigation.
