On April 25 2012 one crew member lost his life in an occupational accident on board the gas tanker BW Suez Everett. The accident occurred in connection with change of mooring wires when the ship was in the Indian Ocean, en route to Yemen.

The AIBN believes that a combination of slack on the wire, a port list and a reduction in friction between the wire drum and deck led to the unsecured drum moved towards the port side and hit the ordinary seaman. Prior to commencement of work, a risk assessment was prepared according to company policy. The AIBN, however, is of the opinion that there were weaknesses related to this procedure, and in addition challenges regarding communication. After the accident the company changed their procedures for risk assessment and routines for safety meetings and the AIBN therefore do not propose any safety recommendations in connection with this accident.
