Marine report 2014/02

Svealand was docked at Tau in Rogaland on the 3. April 2012 for loading cargo, when one of the crew fell 5 meters when he attempted to access the quay using the cargo lift. He died 13 days later from the injury. AIBN has uncovered that use of cargo lift both for cargo purposes and people transport to and from ship without risk assassment or related to other access to the ship. Safety recommendation

Safety recommendation MARINE No 2014/01T

The investigation after the occupational accident on MS Svealand on April 3 2012 has uncovered deficiencies with the shipping company’s ISM regarding safe passage between ship and shore. The cargo lift was used as access to and from shore without risk assessment or measures taken.

The Accident Investigation Board Norway recommend that Eidshaug Rederi AS perform risk assessment for safe passage between ship and shore, and ensure that measures are implemented in the ISM and performed by the crew.
